Long before that reunion, the episode opens with a pair of concurrent bedroom scenes. In one, Ray Donovan is having sex with mayoral candidate Anita Novak. He orders Sam to get out of the mayoral race, knocks her out with his gun, and leaves the room, just as Anita leaves the bed she just shared with Ray. When Sam stirs back to life, she calls Ray. Sam is already drinking and woozy. She could be concussed. She orders Ray to get to the bottom of it, and he races off while Lena stays to keep an eye on Sam. He calls Bridget and asks her to help before he starts training. After an unexpected scene between Sam and Lena—two characters often used primarily as devices—we cut to Ray Donovan confronting the mayor of New York City in a health club bathroom. The mayor tries to play tough guy, but Ray literally throat punches him before he can finish his sentence.

Through these women, Ray Donovan draws on social issues around breasts, sexual identity, motherhood, breast cancer care, and postpartum depression. A running theme of the show this season is motherhood and all of the complicated issues that come with that. What was the impetus behind choosing such a theme? But this was the right time to start to look at issues that are not just impacting women but how men see women and how certain elements not just motherhood… A lot of the focus I was working off of was the simple idea of the female breast and the objectification of it, the violence against it, the use of it, the confusion around it, and certainly this sort of new high level of medical interference with opinions about what a mastectomy is and is not.
